I see a lot of patients with polycystic ovary syndrome. However I also see a lot of women who think they have PCOS but they really don’t! Multiple follicles on an ultrasound is NOT definitive of a diagnosis for this condition. It can be heartbreaking to be labelled with a condition that you really do not heave. PCOS is characterised by lack of ovulation, either in conjunction with amenorrhea (no period) or oligomenorrhea (irregular periods). There also needs to be some evidence of male excess hormones (blood test or clear physical symptoms (hair loss, body hair). Around 70% of women with PCOS will have insulin resistance which is the reason they are not ovulating. Careful detective work is required to work out the trigger for the remaining. As you can see whether you qualify for this diagnosis will dramatically affect direction for treatment. The OCP is not a solution as a pill bleed is not a true period but a withdrawal of hormones. Once the OCP is ceased the condition will remain (and worse in some instances. Treatment needs to be individualised.
